You may administer a readiness test as often as you like.  However most teachers test once at the beginning, once at the end and many also test in the middle of a course.  Nearer the end of the course, it is recommended that you administer the test for the subsequent course in the math sequence.

For example, in a Geometry class, the Geometry Readiness test GR45A19, is most often administered in the first semester, and often in the middle of the school year as well.  Then near the end of the course, the teacher will usually administer the Second Year Algebra Readiness test, SR40A19.
